From my personal experience I can tell you this. First time is always a bit difficult, but once you paid and you enter the cinema, you quickly wonder why you were so stressed about it. Most visitors are men. Straight, bi, and gay. Often middle aged like myself, but also young and elder can be present. And sometimes a woman or a couple enter. If that happens, mostly this site will inform about that. Chances are big that you will be confronted with nudity of other visitors. Some are totally naked, other are just playing with themselves and others are fully clothed. It depends on what they want to do there. Couples or women might engage in sex with each other or with other visitors. If you want to participate, always ask permission first. No touching without asking would be my advise. Sometimes a visit can turn out to be quite exciting. Sometimes not. Some couples notify that they will come. Then there are more visitors, mostly men. Best way to find out is to just try it.
